Birth Certificate Changes And AVSS Version 2007
The VS-10D Certificate of Live Birth
will change on January 1, 2007. The
revisions are necessary to conform to
The following six new data fields will be contained on the
revised Certificate of Live Birth and will be added to AVSS prompts for the
Hospital Birth Record suboption:
20. Date Father Last Worked (MM/CCYY)
23. Date Mother Last Worked (MM/CCYY)
25AA. Date Of First Prenatal Visit (MM/DD/CCYY)
25BA. Date Of Last Prenatal Visit (MM/DD/CCYY)
26A. Obstetric Estimation Of Gestation At Delivery
(Completed Weeks)
26B. Hearing Screening (AVSS List Of Choices)
One existing data field (5E. Planned Place Of Birth) will be
deleted from the Certificate of Live Birth and from AVSS prompts.
Sixteen additional new fields will be inserted into the AVSS
prompting sequence after the SSA1/SSA2 prompts, which now follow the 24C (City
of Mother’s Residence) prompt. These sixteen new fields are not contained on
the revised Certificate of Live Birth:
MSTREET. Mother’s Alternative Mailing Address (Street and
Number, or PO Box)
MCITY. Mother’s Alternative Mailing Address (City)
MCOUNTY. Mother’s Alternative Mailing Address (County)
MSTATE. Mother’s Alternative Mailing Address (State)
MZIP. Mother’s Alternative Mailing Address (Zip)
WIC. Did Mother Get WIC Food For Herself During Pregnancy
(Y, N, U)
CIGPN. Average Number Or Packs Of Cigarettes Smoked Per Day
Three Months Prior To Pregnancy (#C, #P, U)
CIGFN. Average Number Or Packs Of Cigarettes Smoked Per Day

A new
version of AVSS, called 2007, has been programmed to prompt for the 22 new data fields
for births occurring on or after January 1, 2007. Version 2007 will be deployed
during December 2006. The changes will be transparent to users when creating
birth certificates with birth dates before January 1, 2007. The
existing VS-10A Medical Data Supplemental Worksheet (Front Side) and (Reverse
Side) will continue to be used for 2007 births. The VS-10B
Race/Ethnicity and Education Worksheet will be eliminated, but the lists of
choices, including new reporting values for education, will be reproduced on
the Reverse Side
of the VS-10D Certificate of Live Birth.

County Letter ACL06-12. Local Registrars should be careful to include
the correct year and numbering sequence when adding the Local Registration/File
Number (LFN), and if using AVSS LFN Assignment, to test to ensure proper
vertical alignment before printing the LFN. The importance of restarting the
numbering sequence in 2007 is stressed in All County Letter ACL06-17.
