Problem: When a temporary password is assigned by a System Manager, the user, when logging on again must change the temporarary password to a permanent one. AVSS will automatically go to YOUR PASSWORD option.  Many users are confused as to how to proceed at this point.

Solution: Instead of going to the YOUR PASSWORD suboption, AVSS should automatically go one level down to the CHANGE YOUR PASSWORD suboption.

Solution: For item 7 we have UNKNOWN in our list of state country codes and therefore we are allowing UNKNOWN to be printed on the certificates. This will require a recode of item 7 to a dash when UNKNOWN (or it's equivalents) is selected. Additionally, the check for not allowing a dash for field 7 and 8 when 6A,B and C aren't also dashes must be removed.

Problem: In looking at errors occurring on AVSSNet I found that during a re-print of a list to be sent if the user backs out FSLIST3A is not returning to the proper spot in FSLIST3.  Peter has gotten a few replies of users wanting to use the old DOHLIST options since the new one does not loop.

Solution: Modify FSLIST3A to return to the proper spot when backing out of a reprint and operate like the old version for index selection.

Solution: This was introduced by AMR 02068. The HCA,24E subfield check was modified to account for the new format in the ^FSL("ZIP" list but the other forms which access the zip list were not. Modify the CDC (23 and 27D), CMR (ZIP), DMR (ZIP), FDC (28E), FMR (ZIP) and SM (CAZI) forms to reflect the changes made in the HCA,24E,CHK,1 and 6.

Notes: I copied the #HNP1L report to the NP1L report and modified the FORMS section to do both HCA;LCA. It seems to work. Marilyn is going to check the output. Similar to the verification letter, the user's id will control access to the LCA records.

Problem: When breaking out of a report (i.e. using the ^Q at the end of a page) we leave unresolved DO links in the program execution stack. Subsequently when signing off the current session AVSS attempts to process the program execution stack and gets an UNDEF error because we have killed off local variables when ending the session.

Solution: Either clear the stack when exiting AVSS (not sure if this is possible) or check for this condition (i.e. <UNDEF>ERR1+3^FSRP4) and don't log these errors. It may also be possible to take care of this by changing the Quit at line EXIT+4^FSO to a Halt.

Problem: There have been hundreds of DSCON errors on AVSSNet. Approximately 99% occur early in the session (either in the logo display or the answerback processing) but some happen during data entry causing the user to lose what has been entered.

Solution: In the error trap see if we are in the middle of data entry, check to see if the data entered can be filed (i.e. there are enough data items completed to satisfy the primary filing requirements) and file the record as ^Saved at the current prompt. Finally, do not create an error log for this condition - just Halt after filing the record.

Problem: Funeral directors frequently have to travel to a coroner's office to obtain signature in DDC Field 126. This time and effort could be eliminated if the coroner could certify certificates electronically.

Solution: AVSS/EDR allows all authorized participants to remotely review the medical data on draft certificates. This would permit the review and certification by the coroner or deputy coroner without the need to sign the paper certificate. Since the coroner and staff are trusted parties, operating in a protected environment, electronic certification could be accomplished without biometric authentication by means of a password/PIN approach.  The following modifications would be required:

1. Create a Coroner Electronic Certification suboption for the DDC.

2. Allow Coroner Electronic Certification suboption to be accessed only by those individuals having a special Coroner Electronic Certifier classification. Access can be further restricted by special password for this new suboption.

3. Executing the Coroner Electronic Certification would result in setting the value of Field 126 equal to the name of the coroner or deputy coroner corresponding to the encrypted password.

4. A unique electronic certification number (ECN) would be created whenever the coroner certifys a certificate.  This number would be of the format:

FMLxxxx, where F=first initial of coroner's name, M=middle initial, L=last initial, and xxxx=4-digit sequential number, similar to a local file number.

5. The coroner's name and a barcode corresponding to the ECN would be printed in Field 126.

6. When the LRD links the DDC to the CDC there would be a prompt for ECN. If there is no barcode printed in Field 126, the LRD staff just presses Enter. If there is a barcode, however, the LRD staff will scan the barcode and it must agree with the value of the ECN stored in the AVSS/EDR database.

Problem: When hospitals move from stand-alone AVSS PCs to AVSS/NET they no longer have access to historical Hospital Birth Records (HCAs). This has not been a problem in LRDs where hospitals are not stand-alone (ones that dial into an LRD's AVSS computer) since they do not presently have the ability to view historical HCAs or registered birth certificates.  There were complaints, however, when the first (Ventura County) stand-alone LRD migrated to AVSS/NET. In response, the AVSS Project developed a means whereby AVSS/NET hospitals could view the non-confidential portion of registered birth certificate electronic records (LCAs) that occurred at their site.  It was our understanding from OVR that hospital staff would not be allowed to view the confidential portion of LCAs. However, a recent communication (see below) from the State Registrar, Michael Rodrian, states that, with the advent of HIPAA laws, it is now permissible for hospitals to view the entire electronic record for those births that occurred in their institution.

Michael Rodrian’s Response:

I agree with the logic of being conservative regarding hospital access to registered certificates. In any case, no hospital should be able to print out a registered certificate.  However, as long as a hospital is absolutely restricted to viewing data it has submitted, I am confident that both vital records laws and HIPAA laws prevent a hospital from making an un-authorized disclosure. Since a hospital has reported the data to us, it is evident to us that they were authorized to have it.  Therefore I do not see a problem with them using the AVSS system to call up this data for purposes connected with a parent request, or to improve the health and safety of the mother or child, or to improve hospital health practices.  Again, they remain bound by both vital records requirements and HIPAA requirements of confidentiality and privacy.

Solution: Add DISPLAY CERTIFICATE for the LCA to all hospital user

classifications at all AVSS/NET Uci's.
