Problem: When we produce the preliminary NCHS files we report on Type of Place of Delivery (column 21 in the layout). There is a table that maps Hospital Codes to the type.  The table is stored in ^FSL("NCHS95","TYPE",hcode)=type.  David reports that there has been an increase in the number of unclassifiable places (from 3 in year 2002 to 137 in 2003).

Solution: When new facilities are added to AVSS we must update the table with the proper recode value. There are currently 4 hospital codes without a mapping: 781, 782, 783 and 784. The codes possibilities are:

1 - Hospital; 2 - Freestanding Birth Center; 3 - Office; 4 - Residence; 5 - Other; and 9 - Not Classifiable

A recent Help Desk request from Lorna Backus in Santa Barbara indicates that this list is too limited for coroner use. In the opinion of Alan Oppenhiem (see below), the 2003 CDC specifications will be relaxed for this field to allow free text and there will be no requirement for AVSS to do the coding.

Solution:  As per the suggestion of Alan Oppenheim, allow free text (length=90) for DDC/CDC Field 123 and remove the internal coding associated with this field (Field I123).

OK, let's go with free text entry for EDR item 123, and let SuperMicar do the Place Of Injury coding (just as it does the Cause Of Death Coding). I will remove the 123 codes from the CDC Specs, and I will remove I123 from the EDRS file layout spreadsheet.

CA,OTHER STATE   CA,PASADENA CITY   CA,TEST COUNTY   CA,UNKNOWN

2. We may want to check for use of the TEST COUNTY entry in the HCA,24B subfield check so that it, too, is not allowed during data entry. Should we remove this value from the ^FSL("CNTY","CA" list?

Problem: System did a backup at 4:00 am when it should not have.

Solution: This is probably the first time that this has happened. The time job happened to be checking for doing a shutdown at EXACTLY 4:00am ($H time of ,14400) with a start time greater than the stop time. At SSD+7^FSPCTIM we check for $H time less than STOP but don't check for the case when $H is equal to STOP. So the fix is to change the 'less than' to a 'not greater than' when comparing $H to STOP.

Evaluation: It was also necessary to modify FSCMRWP1 to add a ! after a # before the Section D info because Caché did not reset $X and $Y positions when a $C(12) is written.

There are several other routines that send # to printer ports which we may want to fix in the future. They don't seem to be having a problem now.

They are: FSCTRSL FSCTLCTZ FSCTLST1 FSCTLSTM FSCTLSTR FSCTLZCT FSLIST3 and FSDI.

I think FSDI does not exhibit the problem because we use the CLOSE code form the device table. An indication of the problem is the appearance of the letters HJ where there would typically be a form feed.

Problem: When the birth certificate was revised to allow multiple race reporting, the space for printing 20C and 23C was reduced so WITHHELD had to be abbreviated to W/H. Fields 20C and 23C were for the BCA form were not reprogrammed to accept this abbreviation.

Solution:  Program Fields 20C and 23C of the BCA form to accept "W/H"

1. The value 'W/H' is now accepted by the HCA form as well.

2. Since the BCA inputs values from the CBC tape, 'W/' is now accepted

since the length of fields 20C and 23C is 2. Thus W, W/, W/H, and WITHHELD

are now all acceptable for education.

3. 'UN' is also accepted for BCA/20C and 23C.

Problem: When systems were moved to Cache AVSS started leaving item 16 null. This is because this item is not prompted for - it is set by the %INIT code whenever a new LCA is being created. The %INIT code had not been updated to look for the default value in the ^FSU("INSTALL" area. I've modified the %INIT code to use the proper values for future records. In addition we have had a few requests for LRD's wanting to set their default. Glen was using FSFORM to update the entry not realizing that in Cache modification to the forms are reflected across all the LRD namespaces. So in addition to the nulls we also have some incorrect values stored for item 16.

Solution: I have written a fix routine to go and set the appropriate values into the AVSS database.

Problem: There has been a persistent error which occurs during data entry but then goes away when AVSS automatically repropmts. During checking the Cache status one user, Carol Elmore at Oroville hospital, finally provided the clue to discovering the circumstances under which the error occurs. She said that she accidentally hit the F10 key (^V). Now I can reproduce the error. From there I discovered that the FSHCAVL3 routine is setting the value of the variable VAR, which is not a good idea during data entry.

Solution: I modified the routine to use a different local variable and the error no longer occurs.

IN THE DRAFT DEATH CERTIFICATE PLACE OF DEATH (BOX 101), WHEN THE "MISSION GARDENS SKILLED CARE" NURSING HOME IS TYPED IN AVSS AUTOMATICALLY CATEGORIZES IT AS A HOSPITAL.  THIS MEANS THE MORTUARY HAS TO CHOOSE FROM THE HOSPITAL "INPATIENT," ETC. INSTEAD OF BEING ABLE TO CORRECTLY IDENTIFY THAT FACILITY AS A NURSING HOME.  PLEASE CHANGE THE FACILITY "MISSION GARDENS SKILLED CARE" TO A "NURSING HOME" CATEGORY.

2. DR. JEFFREY C FRIED HAS CHANGED HIS ADDRESS FROM ARRELLAGA ST.

TO: 4 N-05,P.O.BOX 689

IN THE DRAFT DEATH REGISTRATION SYSTEM THE FINAL (NOT A "DRAFT" COPY)DEATH CERTIFICATE CAN BE PRINTED WITHOUT HAVING BEEN APPROVED BY THE LRD.

THE STEP INVOLVING APPROVAL BY THE LRD HAS BEEN CUT OUT. 

IF A MORTUARY TRIES TO REGISTER A BURIAL PERMIT BEFORE PRINTING THE FINAL DEATH CERTIFICATE FORM, A MESSAGE APPEARS STATING THAT THE DEATH CERTIFICATE HAS NOT BEEN APPROVED BY THE LRD... AND THE SOFTWARE WILL NOT ALLOW THEM TO REGISTER THAT PERMIT.

THE MESSAGE SHOULD READ THAT A FINAL DEATH CERTIFICATE FORM HAS TO HAVE BEEN PRINTED BEFORE A BURIAL PERMIT CAN BE REGISTERED, BECAUSE AVSS CUT OUT THE EXTRA STEP OF REQUIRING LRD APPROVAL BEFORE PRINTING A FINAL DEATH CERTIFICATE.
