occurred at all on any lines.

for us to edit this field after the record has been transferred over to the statistical file?

verification does not take place and no warnings are issued if a difference occurs. Entries

original values for 3 and LFNV are unaltered.
Implement the same verification checks, along with the same consequences, as found in
the birth certificate.

Problem/Suggestion:
On HCA, after entering CAUCASIAN in fields 18 and 21, while still in data entry, the
internal race codes are correctly coded to "11". When the record is filed, the internal
race codes (I18 and I21) are changed to 40.
Evaluation: The parsing algorithm needs to be corrected for this problem, but in so doing,
it can be improved for combinations of races as well, so as to eliminate the need for
manual coding. This would make AVSS race codes more consistent with CBC speci-
fications. These changes are incorporated in V4.5D in the routines FSHCAVAL,
FSHCAVL2, and FSHCAVL3.

Problem/Suggestion:
It would appear that DMR records get transferred more than once. Quite often, DMR's will
remain on the system for years. I recently backed up all DMRs prior to 1/1/94. At the
present time, there are DMR records for 1994, 1995, and 1996. I checked some of the
DMRs to see whether or not they had actually been linked and reported as CMRs. The
outcome indicates that, based on my limited sample of 10 records, each of the DMRs was
linked and sent to the state.
I need a utility, similar to the one used with HCAs and LCAs, that matches a DMR record
to a CMR record, and prints sufficient information to allow the DMR to be deleted.
This utility could display the matched records on the screen and inquire as to actions
to take: i.e. Do you want to scratch this DMR? (N).
I suppose an alternate solution is to let DMRs age one year, based on date received, and
system date. After one year, each record would be deleted automatically, via a batch
utility.
Evaluation: Suggest developing and testing AMR 96-062 first. Meanwhile, could use KILL RECORD
DATA with a date range.

I really like the new attendant editor in the hospital database area. While using it a few
things happened I thought you would be interested in. While in "EDIT" mode, after
a search has completed and a hit is presented the question "IS THIS THE
ATTENDANT <Y>" is asked by AVSS. An "N" response is not understood yielding
the following response, "INCORRECT RESPONSE". It would be convenient to have
"N" recognized as I do not always want to acknowledge a correct hit in order to keep
searching for other names.
Also, while entering new attendants the system is not checking for existing names. I was
able to enter the following attendant which already existed in the attendant list:

BIRTH CERTIFICATE OPTION, CBC TAPE GENERATION (1400)
The current version of this tape program eliminates complete records from the output
tape under certain conditions. The most prevalent condition, based on actual output from
the procedure, appears to be length errors for certain items. These items are identified
by numbers: 36,47,84,94. Item 94 seems to be the one that occurs most often.
Proposed solution: Set errors to an agreed upon value depending on the type of data
that is receiving the error. For example, if the variable is numeric, and a length error occurs,
then set the value to all 9's or leave it blank. If the variable is character, then set it to an
agreed upon number of "-" (dashes). Or simply, leave all fields with errors as BLANK. At
least the remainder of the record will be available for analysis. This will place more
records on the output tape, and will only exclude certain records from being analyzed.

I am suggesting an AVSS modification in regards to twin or multiple birth processing.
It would be very helpful if after the 1st child was entered, AVSS would recognize the
multiple and prompt for the name of the 2nd child, and the pertaining information for
field #27 only, then duplicating the mother and father's information by AVSS, eliminating
the need to type in the same information twice by the processor.
Evaluation: Do not recommend due to complexity vs. potential time savings. Tried once in early
AVSS with poor results.

Problem/Suggestion:
CBC Output File: I noticed the last release of AVSS omits writing records with skipped fields
when creating a file using the new CBC option. I think that forcing the user to complete the
record before being able to output it to a file is overkill, especially since, at least in my experi-
ence, the incomplete fields are usually not fields I am interested in using for analyses. Further-
more, every record that has been excluded from my attempts to create a file have been
reallocates. Often we don't have the information to complete the record. I believe the report
that is generated when the job is run in the background, identifying the records and associated
skipped fields is very useful for identifying records that need to be completed. Users
interested in completing the records will use this list to do so. However, I don't think the
user should be restricted from exporting a record because of one or more skipped fields. In
San Bernardino County we use the CBC file to backup the data to the mainframe and for
analyses. It would be preferable to include all registered records for these purposes.

Problem/Suggestion:
Last week (April 25th) our AVSS database ran out of space while data entry was in process
coupled with reallocations being received. The result was a corrupt, irreparable database.
Fortunately, we were able to successfully transfer the records that had been entered that day
and the reallocates received that morning to the backup from the previous day.
This situation suggests that the current method of issuing a warning when available blocks
drops below 1000 may not be sufficient, especially since no warning is issued until you
leave AVSS and then return. What must have happened to us on April 25th is that when the
clerks logged on in the morning there were more than 1000 free blocks, so no warning would
have been given. Two clerks began entering data and at the same time the reallocation process
was running (and we were receiving records). The sum of this activity used virtually all of the
remaining space yet there was no warning until a clerk logged off and then returned to AVSS.
I suggest the size of blocks that trigger the warning be changed to a percentage, say 5% for
each site, given the broad range of database sizes between LRD's.
Note: I realize one could argue that the hardware (e.g. disk) on the main server was bad and
not the database. However, I copied the entire database to our AVSS backup machine and
was unable to expand it on that machine either. I then restored the backup database from the
previous day to the main server and the database expanded without a problem).

mately 80-85%. This leaves a large portion of our records with no ctract. In order to
expedite the correction process, we create an ascii file from the FSREJ global. This file is
then sent to another section, where the rejects are corrected and actual census tracts are added
to the ascii record. Attached is a sample printout of the file, indicating where the LFN and
Ctract values are located in the ascii file.
Solution: Develop a generic utility that will allow the user to specify the location of the
LFN (index) and the Ctract value in a named ascii file and then to read that file in order
to update the census tract for the record in ^FS indexed by LFN.

Problem/Suggestion:
Problem: SENT1 is presently ambiguous and has virtually no useful purpose because:
It is set only on the sending computer.
It is set again when LCAs are transferred from the LRD to OVR. This is because
it was originally designed to determine when the hospital birth record (HCA) was sent
to the LRD. Now that AVSS is transmitting electronic records to OVR, it can have
several interpretations at the LRD and possibly at OVR.
Additional problem: At present SENT in LCA (as displayed at 21,63) wraps around due
to its extreme right position, and it is on a different line than SENT1. Also, as discussed
below SENT1 will be renamed into SENTHCA and SENTLCA and should no longer be
displayed.
Solution:
Set a new date variable SENTHCA at both the hospital and the LRD whenever an HCA
is transmitted (from hospital to LRD).
Set a new date variable SENTLCA at both the LRD and OVR whenever an LCA is
transmitted (from LRD to OVR).
Rename existing SENT1 values to SENTHCA for HCA forms and to SENTLCA for
LCAs.
I believe this means that SENTHCA can be interpreted as the date of last electronic
transmission of the HCA form to the LRD, and that SENTLCA can be interpreted as the
date of the last electronic transmission of the LCA form to OSR.
Additional solution: Move the display for Field 31 of LCA from 19,36 to 19,16. Move
display for SENT from 21,63 to 19,30. Remove display for SENT1. Display SENTHCA at
19,46 and SENTLCA at 19,62. Make corresponding adjustments in HCA as well, except omit SENTLCA. (Also, print coordinates for DATEAB - see AMR 96-080)

Problem: California law will require information on the marital status to be entered on
electronic birth certificates.
Solution: Program Version 4.6 to include the following question:
MAR. FATHER AND MOTHER MARRIED?
With the same response (Y,N,U) but with new help instructions to be provided soon.
This prompt will now immediately proceed field 6A (first name of father).
Note1: Variable name is now MAR, not MMAR and internal value will be IMAR not
IMMAR.
Note2: Alan will send new coding rules and help instructions.
NOTE TO AVSS PROGRAMMERS: DO NOT START PROGRAMMING UNTIL WE GET
FINAL SPECS FOR THIS AND THE FOLLOWING (THERE MAY BE CHANGES ---

Since the bill will likely become law before we begin implementing Version 4.6, we should
go ahead and insert the prompt (after SSA2 and before NEWS) in HCA. Alan thinks we
can just implement it as we update the sites, i.e., it will not have to take effect simultaneously
at all sites on January 1, 1997 --- however, he will check to be sure.

Problem: The standard report #HLATR1T (Late Registration Report) has no meaning at the
HCA level since it compares the time delay between Fields 4A and 17, but Field 17 is not
yet available at the HCA level.
Suggestion: Delete #HLATR1T and add #HLATR2T (Late Submission Report) which will
be identical to #FLATR2T, except it applies to the HCA form.

Problem/Suggestion:
The problem of how to deal with out-of-hospital births has been a recurrent problem in the
registration of birth records. Often there are problems discerning the licensed status of
certified nurse midwives and/or lay midwives as attendants at the births (boxes 13A &
13B). One proposal could be to have the Board of Consumer Affairs (Nursing board section)
provide the Office of Vital Records (OVR) with an annual updated list of all licensed CNM's
and lay midwives that are currently licensed in this state. If OVR staff could then take on
responsibility of doing the data entry on these individuals (with the assistance of AVSS
programmers), maybe we could better track this process and ensure proper information on
the birth certificates. This option would not eliminate others such as paramedics, spouses,
or any other individuals in attendance from signing when necessary. However, with
a statewide list (in AVSS), it might make matters easier for everyone involved in the birth
registration process.
Evaluation: Depending on the size of the list, it could be difficult and confusing. Needs TAG
discussion.

Problem/Suggestion:
PROBLEM: NCHS needs California's electronic birth certificates on a more timely basis.
SOLUTION: Create new option at OSR (only), called "NCHS FILE GENERATION" to
generate a NCHS file using the same procedure as presently exists to send a CBC file to Data
Systems Branch. A new unique sequential number (SQN for sequential number?) would be
written to the file in place of State File Number. A new global (name?) would be created to
allow records to be indexed by SQN. NCHS file would be written to disk on OSR computer,
not to network. This file would be created monthly, zipped into a self-extracting *.EXE file,
and forwarded to Ron Chamblee via Wonder/PC.
Note1: I have attached NCHS.EXE, a self-extracting file of 1000 NCHS records. Unzipped
file was 257kb, zipped down to 36kb, and was 51kb as an EXE file. This suggests that the
monthly EXE file would be about 2mb, which is reasonable to transmit via Wonder/PC. Ron
Chamblee's Wonder address is RFC2.
Note2: Alan will investigate RFC2's statement about "…all data items, not just current flow
items --- will be transmitted".

Incorporate the following (provisional) instructions into HELP response for DECP:
A VOLUNTARY DECLARATION OF PATERNITY IS REQUIRED IN ORDER FOR THE
FATHER'S INFORMATION (FIELDS 6A-8) TO APPEAR ON THE BIRTH CERTIFI-
CATE. THE BIRTH CERTIFICATE MAY BE AMENDED TO ADD THE FATHER'S
NAME AT A LATER DATE ONLY IF PATERNITY FOR THE CHILD HAS BEEN
ESTABLISHED BY A JUDGEMENT OF A COURT OF COMPETENT JURISDICTION
OR BY THE FILING OF A VOLUNTARY DECLARATION OF PATERNITY. A YES/NO
RESPONSE IS REQUIRED: THIS QUESTION DOES NOT HAVE "WITHHELD" AS A
VALID RESPONSE. ONLY IF THE PARENTS ARE UNAVAILABLE SHOULD YOU ENTER "UNKNOWN".

Problem/Suggestion:
Problem: Using the existing DMR form in the district, there is no easy way to determine
whether or not an individual case has been followed up and completed by nursing.
Proposed solution(s):
Option #1: Add two (2) additional date fields to the DMR form: Date Sent to Nursing
(DATNURS) and Date Epi Ret'd from Nursing (DATRETD). Develop a simple line listing
report which looks for missing dates in the DATRETD field. This report could be entitled
"District Outstanding Case Report" or CD Overdue List.
Option #2: Alternatively, it might be more standard to develop the List to be Sent to HD
option for the DMR forms. If this option were functional, then we could define outstanding
cases as those whose SENT2 date were undefined. This would yield similar results to the
first option.

Problem/Suggestion:
In the HOSPITAL BIRTH RECORD OPTION, in REGISTER BIRTH, field 19, and 22, the
Hispanic Father, and Mother, has the word 'HISPANIC' listed as a possibility. Patricia
Andrews at the state says that it is not allowed and the certificate returned if that is response on
the certificate. She said that if the parents list themselves as 'HISPANIC' with no specific
nationality then the clerks are to put 'WITHHELD' in 19 and/or 22.
In the fields 19 and 22, take out the word 'HISPANIC' from the list as a possible response
to those fields.
